The Service offer will be agreed between the client and OT
The payee will handle all tax and national insurance.
The therapist must be notified within 48 hours for cancellation. Failure to do so will result in this session or days work being charged in full or deducted from the total sessions.
I am bound by the Codes of Ethics and Practice of the Royal College of Occupational Therapists. The clinician has the right to refuse duties that are not within the standards and ethics outlined. The normal duration of each session is 50 minutes although I reserve the right to amend that time for therapeutic reasons. If for any reason the attendee arrives late to a session, they will be seen for the remainder of the session but not beyond the allotted time as this will disrupt the sessions for other students who may be waiting.
It is understood that sometimes sudden unexpected events or emergencies may occur that may make it necessary for appointments to be cancelled at the last minute or that may cause failure to attend where prior notification is not possible. On these occasions it is at the therapist’s discretion if a fee will be charged. In general, however, failure to give less than 48 hours’ notice of your intention to cancel or postpone an agreed therapy input or if there is a repeat pattern of cancellations/DNA’s the therapist reserves the right to charge in full for that day’s input.
Payment should be made at the end of each month. Please note for payment, my Sort Code is 601328, my account number is 47181796 and my account details are Natwest Bank, Luton Market Hill Road, 31 George Street, Luton. These details will also be provided within a monthly invoice.
Invoices will be provided no later than the date of the agreed appointment.
Services must be paid within 28 days of invoice late payments will incur a 10% surcharge.
Services may be withdrawn at any time if payment remains outstanding.
The cost of therapy includes any written materials supplied but excludes the cost of any books that have been suggested by the therapist as recommended reading, or the purchase of any recommended equipment.
As part of the code(s) of practice the therapist is required to carry out continuing professional development, and to engage in regular on-going clinical supervision. This is to ensure an ethical and professional service to clients. If any cases are discussed in this clinical supervision, no identifying details will be disclosed.
Confidentiality will be maintained within the codes of ethics and legal requirements. Confidentiality does not apply where it would mean that I, as your therapist, might break the law or where withholding information means I would breach the codes of ethics. Confidentiality from individual therapy sessions may be breached if I consider there is a risk of harm to the child or others. In such exceptional circumstances, where there is concern for well being, it may be necessary to seek help outside the therapeutic relationship. In such an event where I am considering breaching confidentiality, the service user will normally be consulted first.
In the case of a disclosure concerning acts of terrorism, vulnerable adult or child protection issues or drug trafficking, confidentiality will be breached and such disclosures will be passed onto the relevant authority without delay. Due consideration should be exercised before disclosing anything of a previously unreported criminal nature, as I am obligated to contact relevant authorities.
The therapeutic relationship with service users will remain a professional one at all times.
Notes may be taken during and after each session, which will be kept in accordance with the Data Protection Act (1998). These notes will be securely stored. I will discuss the disposal, retention or otherwise of any such notes at the end of our engagement. They are disclosed to no one other than the clinical supervisor, unless required under a court of law subpoena. You have the right to inspect your records should you so wish, and this request will be fulfilled during a therapy session.
I will not suddenly or without warning terminate the contract of therapy, except in exceptional circumstances, which would become clear in the course of the work together. This would be fully discussed at that time. Please note any threats or acts of violence will invalidate this agreement and therapy input will cease.
You will be notified of any holidays to be taken by myself well in advance. However, there may also be occasions when sessions may be cancelled due to illness, injury or attending training sessions or meetings. I will try to give you as much notice as possible of any cancellations and will offer an alternative time. Therefore, please notify if there are ever any changes in contact details.
In the event of you being unhappy with the service you receive, please discuss this with me. If you feel unable to do so or do not receive a satisfactory resolution, then you have the right to complain to my professional body RCOT.
Accidental injuries can occur during sessions and the therapist will not be liable for this. However, a suitable, safe environment will be provided for the therapy sessions and suitable provisions made to be able to carry out any other work required by the service user.
